Finalize Your Health Forms and Insurance Information
Completing this health information is essential for your health and the health of our community.
Health Insurance
Meredith requires health insurance for every student. This requirement can be met by an approved policy that the student already has or by purchasing the BlueCross BlueShield (BCBS) policy made available through the College.
If you already have insurance, you must verify that it meets the college’s minimum requirements and if so, submit a waiver. Have your insurance card handy when logging into the portal.
If you do not have insurance, complete the enrollment now so you can receive your insurance cards promptly.
Please note that a charge for insurance coverage may appear on your bill until you have completed the waiver.
Take Action
Indicate your insurance decision in the insurance portal. The insurance portal opens in June.
You must have insurance coverage that starts by or before August 1 for Fall entry and January 1 for Spring entry.
Health Forms
Every student must complete their health forms by July 1. Failure to complete these forms by the deadline may delay the release of your course schedule.
Ensure you have everything you need for the Student Wellness Portal including your username and password to access your Meredith email.
Please note some of the forms will first need to be printed and completed by your primary care provider.
- Domestic/International Health Department Immunization Record
- High School Transcript or College/University Record
- Military Record
- NC Immunization Registry (NCIR) or Other State Immunization Registry Record
- Meredith College Immunization Record Form
NOTE: All forms of immunization documentation must be signed by a healthcare provider or stamped by a health department.
Take Action
Complete all health forms in your Student Health Portal.
All forms must be completed by July 1 for Fall entry or December 1 for Spring entry.
