3. Ensure your forms are submitted before 3 p.m., Friday, November 3. How can you submit your forms:
The deadline cannot be extended. All forms require a hand-written signature (the document may be scanned, but a written signature is required). Form fillable PDFs do not work if you open them in a browser window (e.g. in Edge, Firefox, Chrome). Always download the PDF form to your computer before typing to enter information.
