Open Enrollment Deadline Reminder

Please don’t miss the deadline for Open Enrollment 2024! Benefit elections must be received by Friday, November 3, at 3 p.m.

  1. Review your options: The Open Enrollment page is now available.  It includes the 2024 Benefit Guide to give you an overview of all of your benefit options; and the 2024 Open Enrollment Newsletter with 2024 rate information and details about each plan, as well as links to access forms; and

2. The Benefit Checklist to give you a quick review of all of the options available to you. 

  • Submit forms for required annual re-enrollment: Re-enrollment is required each year for the following, even if you don’t wish to make a change:
  • A Health Savings Account (HSA medical spending account);
  • A Flexible Spending Account (Full Purpose FSA, Limited FSA or Dependent Care); or
  • The Health Insurance Opt-Out  (please provide a copy of proof of current alternative health insurance coverage).


3. Ensure your forms are submitted before 3 p.m., Friday, November 3. How can you submit your forms:

  • Email (;
  • Fax (#8164);  
  • Hand-delivered to the Office of Human Resources, first floor Park Center; or
  • Interoffice or US Mail – please consider delivery time to ensure they are received before the deadline. 


The deadline cannot be extended.  All forms require a hand-written signature (the document may be scanned, but a written signature is required). Form fillable PDFs do not work if you open them in a browser window (e.g. in Edge, Firefox, Chrome). Always download the PDF form to your computer before typing to enter information.
