The policy year is from August 1, 2010 – July 31, 2011.   United Healthcare mails brochures to all eligible students in the Summer.

Policy highlights for the optional injury and sickness plan include:

       • United Healthcare has a large network of providers in the Triangle area.

       • $50 deductible per insured person per policy year for Preferred Providers

       • $100 deductible per insured person per policy year for out-of-network providers

       • If prescriptions are filled at a UnitedHealthcare Network Pharmacy (UHPS) the following copays
          and limits apply:

                   o $15 copay per prescription for Tier 1 for 31 day supply
                   o $25 copay per prescription for Tier 2 for 31 day supply
                   o $750 maximum per policy year

       • Psychotherapy is paid at $75/day with a maximum of 20 visits per policy year
      

       • Coverage effective from August 1, 2009 - July 31, 2010
      

       • Coverage available for dependents

*This plan has a $50,000 maximum benefit for each injury or sickness and therefore is not a
