Health Forms

A student health form must be completed and all immunizations documented and updated, prior to matriculation. Students are not allowed to register for classes until their Health and Immunization forms are completed and returned to the Office of Admissions.

All health forms are due on the specified date on the instruction sheet. All ocular and dental work should be attended to before students enter or scheduled for vacations or holidays.

Student Health Insurance Enrollment Information 2011-2012

Meredith full-time undergraduate students and dietetic interns utilizing United Healthcare StudentResources for health insurance will receive a permanent insurance card mid-September.  The benefit period for the health insurance Meredith offers is August 1, 2011 through July 31, 2012. 

To enroll early and receive an insurance card prior to mid-September, create an account with United Healthcare StudentResources.  Click here, then select "Create an Account".  Information needed to create an account are student name, date of birth, and Meredith e-mail address or Meredith student ID number (found on your Cam card).  Create a user name and password to access information from United Healthcare StudentResources. 

After creating an account, login to the account created and select "Login to My Account" utilizing the user ID and password created for United Healthcare StudentResources.  Options to view claims and print an insurance ID are available here.

For graduate students and part-time students choosing to purchase the insurance Meredith offers through United Healthcare StudentResources, please complete the enrollment form by clicking here.  Then select "Enroll On-line Now" and follow the procedure for purchasing the policy on-line.
