Announcement - Fall 2007
Meredith is happy to announce a new offering of an additional benefit to enhance the health and wellness of faculty and staff. After a successful pilot program this summer, where faculty and staff were seen in the health center for minor illness or injury, a decision was made to continue the program.

Mission
To provide you with the finest professional service in a courteous and safe atmosphere with the convenience of on-site, urgent health care.

Medical Services
The services available are for confidential clinical care for minor illnesses, health education, and minor emergency care. All records at Carroll Health Center are confidential. No one will have access to your medical record without your written consent. To take advantage of this service, you will need an appointment. You may wish to complete the Medical History form prior to your visit to save time.

You may still come by the Health Center to self-check your blood pressure and weight in the waiting room during operating hours. If you are concerned about your chronic care needs, you will need to see your primary care physician for evaluation.

Cost
The cost of a visit to the health center this summer was equal to your co-pay with your primary care physician. After input from faculty and staff, the co-pay was changed to a fixed charge of $20.00.

Co-pay for this service: $20.00 (Please pay with cash or check) We will provide a receipt for flexible benefit reimbursement.

Clinic Hours
Monday - Thursday 8:30 am - 5:30 pm
Friday 8:30 - 5:00

Clinician Hours
Monday, Tuesday and Friday 9:00 am – 10:00 am
Monday – Friday 3:00 pm – 4:00 pm

To see the clinician, you will need an appointment. Please call 760-8535 to schedule.

Inclusions

• Free Self Blood Pressure and Weight Check (In waiting room)
• Upper Respiratory Infections/Viruses including Flu and Strep A
• Urinary Tract Infections
• Urinalysis testing
• Minor injuries
• Free influenza vaccine

Exclusions

• No labs except Strep A and urinalysis
• No EKG’s X-rays, or sutures
• No insurance filed
• No chronic care, i.e. ongoing medical concerns
• Not a replacement for your primary care physician or specialist
• No interpreter on site at the Health Center – employee would need to furnish his/her own interpreter
