Students who utilize Sign Language Interpreters or Cued Speech Transliterators must complete this form in order to make an official request to have an Interpreter or Transliterator present.

Occasionally Financial Assistance will contact our office because they are aware of financial aid that a student with a disability may qualify for. In such cases, we are able to give names of students to FA who have previously completed this form.

Both of these forms must be submitted in order for Disability Services to obtain a student's textbook in an electronic format in order to use Screen Reading Technology.

This form serves as a student's official request for accommodation letters and serves as permission for Disability Services to speak to a student's intructors, when necessary. This form must be completed each semester in order for the student to complete their Accommodation Review Meeting.
