Evaluation of Student Performance:

Evaluation of the student's performance in field and of her readiness to begin practice as an entry-level professional is a critical part of the field instructor role.  It is essential for the student and for the profession that you provide a thorough, thoughtful, in-depth evaluation.  Please complete this task honestly and objectively for the ongoing good of the student and the clients she will serve.

            5        Student demonstrates sustained mastery of and ability to apply in practice generalist social work knowledge
                      values, and skills.  She consistently demonstrates individual initiative and insight that exceed expectations
                      for a senior baccalaureate social work major.
            4        Student demonstrates mastery of and ability to apply generalist social work knowledge, values, and skills.
                      She demonstrates individual initiative and insight that always meet and sometimes exceed expectations
                      for a senior baccalaureate  social work major.
