Official Forms and Transcript Requests

REQUESTING A TRANSCRIPT

Official Meredith College transcripts are issued only at the written request or authorization of the student. When making a writen request, please complete the transcript request form or include the following in your written request:  your full name (including any name you may have used while attending Meredith), social security number, date of birth, dates of attendance, a full legal signature, and the exact address where the transcript is to be sent.

Transcripts are free unless 10 or more are requested at one time, then they are $5.00 each.
