Dental benefits are provided through Blue Cross / Blue Shield of NC.

Eligibility:  Any regular employee who works half-time or more (1,000 or more hour per year).   

Enhanced Dental Plan with Orthodontia:

Lifetime deductible - $100 (all services except orthodontics)

Prior coverage credit

Maximum annual benefit - $1,000

Diagnostic and Preventative - 100% coverage with no waiting period

Basic Services - 80% coverage with no waiting period

Major Services - 50% coverage with 12 month waiting period

Orthodontia - 50% coverage with 12 month waiting period

Employee Only - $34.80 per month

Employee plus spouse - $69.60 per month

Employee plus children - $65.99 per month

Family - $116.16 per month

(all rates may be tax sheltered under Section 125 plan)
