Services are available to any child with a diagnosis on the autism spectrum, who begins services by 42 months of age and lives within a 25 mile radius of Meredith College.

To apply to the MAP, complete the application below and return it by mail or fax. (See contact information below). Once the completed application has been received, you will be contacted and will be asked to set up an observation of the program. All applications and client records, past and present, will be kept confidential.

Parent(s)/guardian(s) understand that the intervention described above may possibly involve the following risks and/or discomforts: (a) it is possible that after six months of intervention, or any time thereafter, the intervention will be terminated and other services will be recommended; (b) although the intervention procedures provided are intended to be beneficial and have helped many children with autism in the past, the child may be unchanged or may experience more difficulties after participation in the intervention; (c) the child may experience some distress when the trainers implement certain procedures, as in ignoring inappropriate behaviors.
